Just got offered a settlement and I have no idea if it's fair

So after almost two years of dealing with this shoulder injury, endless physical therapy, two rounds of cortisone shots, and more paperwork than I ever thought existed, the insurance company finally came back with a settlement offer last week. I'll be honest, my first reaction was just relief that there might be an end in sight, but now that I've had a few days to sit with it I'm starting to second-guess everything. The number they're offering would cover my medical bills and some lost time, but I'm still having issues with my shoulder and my doctor says I might need surgery eventually if things don't improve. I'm worried that if I take this settlement, I'm basically signing away my right to ever get help for this again if it gets worse down the road.
